1. Governmental Entity Letter or Exempt Organization Certificate, containing the
   exemption number
2. Main point of contact (full name), email address, and phone number. 
3. School or Organization Website
4. Billing Address
5. Shipping Address

Our W9 form is available upon request, please mention when ordering.

Note: It may take up to 72 hours to verify information. Once your school or organization is verified, you will not need to provide documentation in order to make future tax-exempt purchases.
